Our trained representatives conduct live post-discharge phone calls to capture authentic resident experiences.

We are a neutral 3rd party with experience in navigating delicate conversations. We want you to have real, actionable data to improve outcomes- and not everyone wants to share that information. Through a series of redlight-questions and open-ended questions, we delve into what is most important to the resident and how to support a better outcome.

Yes! We offer QAPI-ready reports as well as communication in real-time when there is a pressing concern. For example, a resident is struggling at home - the resident can potentially be readmitted to your facility before deteriorating to the point of requiring a re-hospitalization (which hurts your 30-day rehospitalization reimbursement metric.)

Interviews are attempted in English first. If they prefer to conduct the interview in a different language, our team has multiple languages available in-house to readily complete the interview. If the language is not available in-house, then we will utilize our network of professional linguists to complete the interview.

We attempt to speak with the resident first. If the resident is unable or unwilling to complete the interview, then we will attempt to speak with the responsible party / next of kin. If the resident directs us to a different person (family, friend, etc), we will attempt to conduct the interview with that person.

We can begin immediately after signing of contract and receiving resident contact information. You can choose to provide direct access to EHR or to manually send a list of names and phone numbers.

Anytime you would like to cancel the service, it is immediately cancellable and the monthly fee is prorated. Please note that QAPI-ready reports are only available with complete months.

You absolutely can! You can develop the interview technique, questions, and work on limiting bias. However, call us if you are seeking a more robust, objective program that you don't need to actively manage - that includes skilled clinicians who understand:
